Probiotics for Athletes: What the Evidence Says About Gut Health, Immunity, and Performance
Endurance and team-sport athletes are no strangers to gut trouble — cramping, bloating, and urgent bathroom stops during hard training are common complaints, and heavy training loads are also linked to more frequent respiratory infections. Probiotic supplements have become a popular fix for both problems. A 2026 umbrella review in PeerJ (Chen et al.) pooled data from multiple meta-analyses covering 69 randomized controlled trials to see what the evidence actually supports.
What Strains and Doses Were Studied
The trials reviewed used a wide range of probiotic species, most commonly Lactobacillus strains (casei, fermentum, acidophilus, plantarum, rhamnosus), Bifidobacterium strains (lactis, bifidum, longum), and Bacillus subtilis. Dosing across studies ranged from "2 × 10⁸ to 1 × 10¹¹ colony-forming units (CFU)" per day, with intervention lengths spanning 1 to 20 weeks. One representative protocol used "2.0 g/day probiotic capsules containing Lactobacillus acidophilus, Lactobacillus lactis, Bifidobacterium lactis, Bifidobacterium bifidum (1 × 10⁹ CFU per strain)" taken daily for 30 days.
The variety of strains, doses, and durations across the included trials is itself a limitation — the authors note that strain-specific effects remain undefined, meaning the field is not yet at the point of recommending one specific product over another.
Effects on Immune Markers
The clearest signals in the review were on immune-related blood markers:
- Reduced TNF-α, a pro-inflammatory marker (effect size −0.59, 95% CI −0.94 to −0.24, P = 0.001)
- Increased salivary/secretory IgA, an antibody that helps protect mucosal surfaces including the respiratory tract (effect size 0.30, 95% CI 0.03 to 0.57, P = 0.031)

Earlier syntheses referenced in the review also suggest probiotics can reduce the severity of upper respiratory tract infections (URTIs) in athletes, a population that already faces elevated infection risk during heavy training blocks, though this was not measured consistently as a primary outcome across all 69 trials.
Gut Symptoms and Performance: Thinner Evidence
Despite gut health being a major reason athletes reach for probiotics, the umbrella review did not report detailed pooled data specifically on exercise-induced gastrointestinal distress (the cramping, nausea, and GI urgency common in endurance sport). The authors mention only in passing that probiotics "may even enhance performance in aerobic exercise," without quantitative effect sizes — this remains an exploratory, not established, finding.
How Reliable Is This Evidence?
Quality checks using the AMSTAR2 tool rated the underlying meta-analyses unevenly: two were rated high quality, one moderate, and two low quality. Using the GRADE framework, most individual outcomes were rated very low to moderate certainty, largely because of "substantial statistical heterogeneity" between studies (I² greater than 75% for IL-8 and IL-10). The authors explicitly state findings "should be interpreted with caution" and call for future research into strain-specific effects before personalized recommendations can be made.
