The new study: combining training and intermittent fasting

The authors — Jiao, Yan, Zhang, Zhao, Li, and Harun — pooled 65 randomized controlled trials with 3,293 participants total, aiming to find the optimal "dose" when combining exercise with intermittent fasting. Key results include:

The authors propose a practical dose: 45–60 minutes per session, about 4 sessions per week (roughly 230–300 minutes of weekly training), maintained for at least 14 weeks to achieve fat loss and body composition improvements. Mechanistically, the team suggests training promotes fat mobilization and oxidation, while fasting periods force the body to shift toward using fatty acids as fuel — two factors that work together to improve the body's fuel-switching ability.

The role of Zone 2 training

Alongside intermittent fasting, long-duration low-to-moderate intensity training — commonly called Zone 2 (about 60–70% of max heart rate, still able to hold a conversation) — has long been seen as an effective fat-burning training tool. A foundational study in Sports Medicine (San-Millán and Brooks) measuring blood lactate along with fat/carb oxidation ratios in professional endurance athletes versus untrained individuals showed: the athlete group had a clearly wider dynamic range between fat and carb burning, while sedentary or overweight people tended to get "stuck" in a carb-dependent state even at rest — the very signature of metabolic rigidity. Regular Zone 2 training over weeks to months is thought to stimulate mitochondrial biogenesis and fat-oxidation enzymes in skeletal muscle, thereby widening this dynamic range.

What about low-carb diets?

The evidence on low-carbohydrate or ketogenic diets for metabolic flexibility remains not fully consistent. Some animal studies show intermittent fasting improves fuel switching in healthy individuals, but this effect diminishes or is even unclear in already-obese or diabetic individuals — suggesting that the order of interventions (improving fitness and body weight first, then applying specific dietary strategies) may matter more than aggressive carb cutting itself. In other words, low-carb is not a "universal key": for people with existing metabolic disorders, combining regular exercise with dietary adjustments under medical supervision usually delivers more reliable benefits than only changing macro ratios in an extreme way.
