Iron Deficiency in Female Athletes: Why It's Common and How to Manage It
Iron deficiency is one of the most common nutritional issues in female athletes, with studies reporting that a substantial proportion of women in endurance and team sports have low iron stores at some point in a training year. Because iron is essential for oxygen transport (via hemoglobin) and for aerobic energy metabolism, even mild deficiency — before anemia develops — can reduce endurance capacity, increase perceived exertion, and slow recovery.
Why Female Athletes Are at Higher Risk
Several overlapping factors make women who train hard particularly vulnerable to depleted iron stores:
- Menstrual blood losses: Regular monthly bleeding, especially heavy menstrual bleeding, represents an ongoing source of iron loss that male athletes do not experience.
- Footstrike hemolysis: The repeated mechanical impact of running (and to a lesser extent other high-impact sports) can rupture red blood cells in the soles of the feet, gradually reducing circulating iron.
- Low energy availability (LEA): Restricted energy intake relative to training demands — whether intentional or unintentional — is linked to reduced dietary iron intake and disrupted iron regulation, and is more common in athletes following restrictive or vegan/vegetarian diets.
- High training volume: Heavy training load increases the inflammatory hormone hepcidin, which temporarily blocks iron absorption from the gut and release from stores.
Screening: Why Ferritin Matters
Because early iron deficiency often has no obvious symptoms, objective screening is the only reliable way to catch it before it affects performance. A 2025 international consensus statement on elite athlete health recommends that female athletes undergo iron blood profiling at least once a year, using a combination of biomarkers rather than a single number. The recommended panel includes:
- Serum ferritin — reflects the body's stored iron and is usually the first marker to fall.
- Hemoglobin — only drops in more advanced, anemic stages of deficiency.
- Transferrin saturation — helps distinguish true iron deficiency from other causes of low ferritin, such as inflammation.
Using at least two markers together helps clinicians stage the severity of iron deficiency, from depleted stores with normal hemoglobin (the most common stage in athletes) to full iron-deficiency anemia. Athletes should also flag major changes — altitude training camps, sudden dietary shifts, or disordered eating patterns — to their medical staff, since these can accelerate iron loss between routine tests.
Dietary vs. Supplemental Iron
Management should be individualized rather than assumed. The same consensus statement stresses that iron status should be objectively assessed and addressed by a qualified practitioner rather than treated with blanket supplementation, since excess iron intake carries its own risks (gastrointestinal side effects and, over the long term, iron overload).
Practical, evidence-based steps typically include:
- Prioritizing heme iron sources (lean red meat, poultry, fish) and pairing plant-based (non-heme) iron sources such as lentils, spinach, and fortified cereals with vitamin C-rich foods to improve absorption.
- Avoiding coffee, tea, and calcium supplements at the same meal as iron-rich foods, since they can inhibit absorption.
- Using an oral iron supplement only after a confirmed low-ferritin result, ideally guided by a physician or sports dietitian who can select an appropriate dose and formulation, and reassess with follow-up bloodwork.

Female athletes should seek medical evaluation if they notice unusual fatigue, declining performance despite consistent training, breathlessness during previously manageable efforts, frequent illness, or heavy/irregular menstrual bleeding. A doctor or sports medicine physician can order the appropriate panel, rule out other causes, and supervise safe, targeted treatment rather than relying on self-prescribed high-dose supplements.
